
Your First Appointment
Planning Your First Visit
At FDL Dermatology we look forward to working with you and delivering the best medical care possible for you and your family.
To ensure your first visit is as smooth as possible, please:
- Please fill out the new patient paperwork online prior to your first appointment.
- Bring your insurance card and photo ID/for pathology and laboratory testing.

Please make sure your credit card is on file when booking your first appointment as we as we require $100 deposit.
- Bring a translator if necessary.
- Please bring all medical records, including pathology reports, from your prior dermatologist.
If you are a patient under the age of 18, a parent or guardian must accompany you on your first visit.
Please complete the forms on the “Book Appointment” page before arriving for your appointment as a new patient. Be sure to bring a valid, government issued photo ID. We also ask that you bring a current copy of your insurance card as this will be submitted with any laboratory or pathology specimens.
Appointment Policy
We ask that you give us a 24-hour notice if you must cancel or reschedule a medical, cosmetic, or surgical appointment.
Please note that if you cancel your appointment within 24 hours of the scheduled appointment, you you’re responsible for the cost of the entire visit. A 100% charge will also be incurred if the patient does not show up for the appointment.

Accepted Forms of Payment
FDL Dermatology is out-of-network for all insurance plans and is an OPT-OUT Medicare provider. Our office is direct pay and payment is due at the time of service. We offer affordable and transparent pricing with no hidden or additional charges after your visit. If you receive a bill after leaving our office, its not from us. If a biopsy was taken or laboratory tests ordered, FDL Dermatology will forward your insurance information, so any additional invoices would be your responsibility. Same-day appointments are available and we are able to accommodate most patients within a week of their appointment request

Prescription Policy
- After your visit, our office will e-Prescibe all prescriptions to your pharmacy of choice.
We do not:
- Fill new prescriptions based on medical conditions that are described over the phone
- Write prescriptions for family members of our patients unless that person is a patient of FDL dermatology as well
If you are an established patient at our practice, we recommend that you do the following to ensure that you have an adequate supply of medication:
- Ask for prescription refills during appointments
- Keep all appointments, as we do not routinely fill prescriptions for patients not seen within the last year
- Check the medication label to ensure that you are receiving the correct medicine in your name
- Bring your medication list with you to your appointment
Recording During Appointments
We do not allow video or audio recording by patients during appointments.
